| PLAYER
AGE ELIGIBILITY |

|
| For
the 20010/11 season players born from 2005 - 1993 (5-17) can play at
Faustina. |
|
| Some
exceptions can be made for players born in 2006, contact the registrar. |

| COST |
|
|
| House
League (Novice to Midget) registration fee for the 2010/11 season will be
$395. |
|
| Tyke
age players, born 2004, 2005, registration fee will be $295.00. |
|
| Family
discount is offered of $25.00 per player, for two or more players per family. |

can only pay by Visa or Mastercard, sorry no Amex, Debit or mail in cheque
this year. |

and cash at walk in registration only. |

| The
system will ask for your player birth registration number. For Ontario born
players |
|
| it's
the number beginning with the birth year, ie 98-05-123456. If you don't have
an |
|
| Ontario
birth certificate, put in the number
and a short description of what you |
|
| have
for your child. |
|
| |
|
|
| Have
your personal information ready when you go the system, address, postal code, |
|
| phone,
email and an emergency contact name and number for player if we can't |
|
| reach
you. |
|
| |
|
|
| If
you live in Toronto, use Toronto not Etobicoke for your players address. |
|
| |
|
|
| You
can register for Future Stars with this system also, but remember that will
not also |
|
| register
your player for House League if you choose to just sign up for Future Stars. |
|
| |
|
|
| All
on-line player registration go on a
short cycle review list before registration is confirmed. |
|
| To
be confirmed, all information must be provided, payment made in full and the
player |
|
| being
within the age limits for Faustina House League hockey. In addition the
league reserves |
|
| the right to limit numbers by age division
and the number of full time goaltenders accepted, |
|
| space
is limited. |

| |
|
|
| You
will be emailed by the Faustina registrar to confirm official acceptance of
your player into the |
